Financial Responsibility & Payment Policy 

​It is your responsibility to contact your insurance provider regarding coverage, deductibles, and co-pays.​ Payments of any applicable deductibles, co-insurance, or self-pay amounts are due at the completion of an appointment, and may be made only by check, cash, or credit/debit card.
